Community Partnerships

We collaborate with diverse organizations, including community groups, institutions, and hospitals across Chicagoland, to advance our mission. As the aging and dementia hub at the University of Chicago, we work closely with various University of Chicago and UChicago Medicine organizations to uphold a longstanding commitment to community engagement.

Africa International House Senior Art Program

Africa House International is dedicated to uplifting communities by promoting literacy, cultural enrichment, and economic development. It provides educational resources and cultural programs aimed at fostering self-sufficiency, community empowerment, and cross-cultural understanding, especially within African and African American communities.

Age Friendly Englewood Village

Age Friendly Englewood Village aims to support seniors in the Englewood community, helping them to age safely and independently. By connecting older adults with vital resources, the organization fosters a sense of community and interdependence, promoting safe, healthy aging in place. 

Chicago Hyde Park Village

Chicago Hyde Park Village (CHPV) fosters a vibrant, age-friendly community where neighbors support one another, enhancing healthy, socially connected aging. Through volunteer-driven programs and services, CHPV helps members thrive with dignity and companionship, creating an inclusive and caring environment for all stages of aging.

Dementia Friendly Washington Heights

Dementia Friendly Washington Heights is a community initiative in Chicago aimed at supporting residents affected by dementia and their caregivers. Led by a local task force, the initiative offers dementia-specific programs like caregiver support groups, arts programs, and specialized resources at the Woodson Regional Library. This collaboration with local healthcare and community organizations fosters an inclusive, supportive environment for individuals with dementia and their families.

Far South Chicago Coalition

The Far South Chicago Coalition (FSCC) is dedicated to revitalizing and developing Chicago’s Far South Side neighborhoods. Their mission is to foster a thriving community through initiatives focused on economic growth, education, housing, workforce development, and faith-based collaboration. FSCC aims to improve residents’ quality of life and create a unified, empowered community.

Lorenzo’s House

Lorenzo’s House was founded in 2021 by Diana Shulla Cose after her husband was diagnosed with younger-onset Alzheimer’s in 2017. Recognizing the lack of resources for families like hers, Diana envisioned a place that would provide holistic support and connection for those affected by younger-onset dementia. Lorenzo’s House aims to shift the narrative from isolation to connection, offering programs such as Youth Initiatives, Carepartner Connections, Memory Academy, and Respite & Healing Spaces to empower and unite families facing similar challenges.

Peer Plus Health Advocates

Peer Plus Education and Training Advocates is a Chicago-based nonprofit organization dedicated to identifying underserved populations in the Midwest and providing culturally sensitive programs that address essential health, educational, and psychosocial needs. Their mission encompasses offering non-medical education, training, and related workshops to tackle issues such as chronic and preventable illnesses, including asthma, cancer, diabetes, high blood pressure, and HIV disease. With over 22 years of service, Peer Plus collaborates with health centers, free clinics, local hospitals, and social service agencies to meet the challenges faced by disproportionately impacted communities.

Senior Talk with Clara Hubbard

Senior Talk with Clara Hubbard is a media company founded in 2011, dedicated to helping older Americans maintain independence and enhance their quality of life. Through various platforms, including live streams, podcasts, and blogs, they provide news, entertainment, and resources tailored to the needs and interests of the senior community. Their mission is to support and empower seniors by offering valuable information and fostering a sense of community.

South Loop Village

South Loop Village is a volunteer-driven organization dedicated to supporting older adults in Chicago’s South Loop as they age within their community. Emphasizing inclusion and diversity, the organization offers social activities, support programs, and resources that enhance quality of life, enabling residents to remain connected and engaged in the neighborhood they love.

Timothy Community Center

The Timothy Community Corporation (TCC) strengthens communities by providing resources and innovative programs focused on health, leadership, and literacy. Through partnerships and a repurposed community space, TCC fosters learning, healing, and empowerment, supporting individuals and families in their journey toward personal and communal growth.
